How we can help.
- The service provides personalised care and support and improves access to local support services to help people with cancer to lead as full and active life as possible

Extended Access Service
We know it can be difficult for some people to get to our practices during the week and we have a range of appointments available with experienced, trained professionals in the evenings and at weekends including:
- GP: face to face and telephone appointments (Sat /Sun)
- Nurse: providing face to face appointments e.g. for long term condition reviews, smears (Sat /Sun)
- Healthcare Assistant: providing face to face appointments e.g. bloods, health checks (Sat /Sun)
- Long-acting Reversible Contraception (LARC) clinics (Sun)
- Pharmacist: providing telephone appointments e.g. medication reviews
- Physiotherapist (first contact) (Mon-Fri)
- Healthy Minds (support with anxiety, low mood etc) (Mon-Fri)
Our face to face appointments are in 2 of our practices – New Cross and Gibson Lane
In the UK, an average of 30% of GP appointments are for musculoskeletal (MSK) problems. Having physiotherapists work within GP practices and take over these appointments, means that patients get seen quicker and recover faster.
Plus – the workload is reduced for GPs helping to ease pressure off general practice.
First Contact Practitioner (FCP) is a new model evolving UK. It involves placing physiotherapists directly into GP practices and to treat patients who come into the clinic with musculoskeletal problems.

Linking Leads – Social Prescribing
Social prescribing can link you with services within the community to support you to improve your health & wellbeing. They can help with:
- Housing or money issues
- Wellbeing & emotional support – links with community groups
- Loneliness
- Meaningful social activities
What we do
We can link patients with services and activities within their community to improve their social, economic and mental wellbeing by;
- Giving patients an opportunity to discuss what is important to them.
- Link you patients with appropriate services within their community to support them to achieve their goals.
- Provide patients with their own wellbeing coordinator who will help to plan their next steps.
Please contact Linking Leeds to discuss any potential referrals. All referrals will be allocated to the wellbeing coordinator’s working within the relevant PCN.

Planning Ahead Service
Our Planning Ahead Coordinator; can help patients and their families with:
• Recording the views of patients and their families/carers in relation to ‘What Matters to Me’
• Producing a ‘Recommended Summary Plan for Emergency Care and Treatment (ReSPECT)’ for the patient.
• Visiting patients in their homes and talk about what matters to them, power of attorney and will making, funeral arrangements and their preferences for their future care.
• Making referrals to related services and offers a further conversation about resuscitation with a clinician.
Topics that might be discussed during the Planning Ahead Appointment include how you are supported, what local support is available, your preferred place of care and death, Power of Attorney, will making and funeral arrangements.
what is planning ahead?
The Planning Ahead Service records your reflections about what you would like to happen towards the end of your life. The service specifically supports individuals with severe complex or long-term health needs and also people that have expressed an interested in planning for their future.
You can have a chat about your life and future plans with our Planning Ahead Coordinator. Topics that might pop up include; what matters to you, how you are supported, what local support is available, your preferred place of care and death, Power of Attorney, will making and funeral arrangements. You may be given leaflets or be referred to other services. These conversations take place in person in your home with your loved ones present and can be done over a number of visits.
After the conversations are complete, you will receive a ReSPECT (Recommended Summary Plan for Emergency Care and Treatment) form to help inform other health care professionals about your preferences going forward. The ReSPECT form is an important document and Jo will explain its relevance during the home visits.

How to refer?
Patients can be referred to the Planning Ahead Coordinator by professionals or patients can refer themselves or a family member by emailing
wyicb-leeds.planning.ahead@nhs.net with the patient’s name, GP surgery and reason for referral.
